LOS ANGELES – Los Angeles Network for Enhanced Services (LANES) and L.A. Care Health Plan have joined forces in a data-sharing program that will result in better care coordination for L.A. Care members.
LANES has been a health information exchange for hospitals and clinics, but L.A. Care is the first health plan to join the collaborative effort involving health care providers in the Los Angeles County safety net. The new contract will cover L.A. Care’s Medi-Cal direct line of business, representing half of the organization’s total membership.
“We are thrilled to welcome L.A. Care into LANES,” says Ali Modaressi, Executive Director of LANES. “Providers need easy access to the medical records of shared patients to enable coordinated care.”
Medical information for one million L.A. Care members will now be easily shared between L.A. Care and the members’ medical providers, including hospitals and community clinics in L.A. County.
With an aging population experiencing complex medical conditions, care coordination is more critical than ever. Efficient and timely sharing of data is key. We know it can improve outcomes for our members, and that is our ultimate goal.

Care coordination has been shown to improve patient satisfaction and reduce costs. However, without an information exchange, there can be no coordination, so this is an important partnership that will benefit L.A. Care members.
